Health, Dental and Vision Insurance
  • Limited open enrollment – you must enroll within 25 calendar days from your first day of work.
  • Your coverage and premiums will begin on the first day of the month following your first day of work.
  • If you wish to decline any insurance coverage or fail to complete your enrollment within 25 calendar days, then you will be ineligible to elect coverage for yourself and/or dependents until the next open enrollment period (March 2012) or other qualifying event.
  • Your portion of the any insurance premiums will be automatically deducted from your paycheck in two equal installments per month.

Flexible Spending Account

  • Limited open enrollment – you must enroll within 25 calendar days from your first day of work.
  • Your contributions to the FSA program will begin on the next payroll following completion of your enrollment.
  • If you wish to decline participation in the FSA program or fail to complete your enrollment within 25 calendar days, then you will be ineligible to participate until the next open enrollment period (January 2012) or other qualifying event.

401K Investment Plan

  • Continuous open enrollment – you may enroll at any time after your first paycheck.
  • Your contributions to the 401k program will begin on the next payroll following your completion of online enrollment.
